All About Me’ Memory Book

We offer support either in collaboration with the carer or individually with children, resulting with an All About Me book for the young person to keep with a memory box.

Life Story Work

We can also offer a more intensive package: working closely with the child or young person to carry out life story work. This would involve working closely with the childs social worker to obtain relevant documents, photographs and records and is done through a referral to ascertain whether the work is viable.

Health Passport

A handy passport sized booklet used to record vital medical information that many children don’t possess when moving to and from placements.  

Staying Safe resource pack – ‘From puberty and Beyond’

A pack that links in with 2 of the ECM outcomes – Be Healthy & Stay Safe. A pack is given to each foster family and every LAC aged 15+ receives one to keep.

Bereavement Work (My Memory Book)

We have designed a special book that focuses on discussion, refection and compilation of a memory book for a relative or friend.

Gift Bags

We give every young person who joins Chrysalis Care a gift bag, which is age appropriate.

Learning Through Play

We have designed a fun education pack for all Chrysalis Care foster carers to use with their LAC children. 

‘Life Plans’

This is a “child friendly” form of a game that helps children & young people understand and talk about their ‘Care Plans’ and reviews in collaboration with their Foster Carer.
